FOR anyone who has ever sat in KL traffic for 90 minutes, spent a week preparing for a presentation that got cancelled, or tried to get anything done during "deadline season", the idea that stress is quietly doing damage somewhere in the body is not hard to believe.
Most people know that chronic stress is bad for the heart, the immune system and the sleep schedule, but the connection to cholesterol is considerably less talked about.
Can stress actually push your cholesterol into dangerous territory?
Verdict:

TRUE
Chronic stress can raise levels of LDL cholesterol, the kind that contributes to plaque buildup in the arteries, and lower levels of HDL cholesterol, the kind that helps clear excess cholesterol from the bloodstream, though the degree to which this happens varies considerably from person to person.
The primary mechanism runs through cortisol, the hormone your adrenal glands release when the body perceives a threat.
In a genuine emergency, this is useful: cortisol signals the liver to release more glucose and cholesterol to give the body energy for whatever fight-or-flight situation it is dealing with.
The problem is that the body cannot easily distinguish between being chased by something dangerous and being stared down by a quarterly performance review, and so the same hormonal response gets triggered regardless.
If the stress is chronic and the cortisol stays elevated, the liver keeps producing cholesterol at an elevated rate even when there is no physical demand for the extra fuel.
A landmark study by researchers at University College London, published in Health Psychology and tracking 199 healthy middle-aged adults over three years, found that people in the top third of stress responders were three times more likely to have LDL cholesterol above clinical thresholds than those in the bottom third.
This is even after accounting for age, body weight, smoking, alcohol use and baseline cholesterol levels.
A prospective five-year study published in PMC, which followed 234 police officers, found that those in the highest quartile of work-related stress had significantly higher triglyceride levels and lower HDL cholesterol than their less-stressed colleagues, and were 2.68 times more likely to develop metabolic syndrome.
A 2025 cross-sectional study published in Dove Medical Press, which examined 247 university students in Rwanda, found that those under moderate stress had significantly higher morning cortisol levels, higher total cholesterol and higher triglycerides compared to those under low stress.
There is also a short-term effect that is worth understanding separately.
During an acute stressful episode, the blood can temporarily lose fluid to the surrounding tissues, concentrating all the components in the bloodstream, including cholesterol, and making levels appear higher than they are when measured on a normal day.
This haemoconcentration effect, documented in PubMed research on mental stress and plasma volume, partially explains why cholesterol tests taken during particularly stressful periods may look worse than usual.
Stress also works its way through behaviour as much as hormones.
People under chronic stress tend to eat more comfort food, drink more alcohol, exercise less and sleep poorly, all of which independently raise LDL and lower HDL in well-documented ways.
A study of 5,000 workers published in the journal Medicine found that even mild job-related psychological stress was linked to elevated LDL and lower HDL, but that workers who did physical labour showed more normal cholesterol levels despite similar stress.
This suggests that physical activity may help offset at least some of the lipid effects of stress.
Now for the BUT.
Not everyone's cholesterol responds to stress in the same way, and the UCL study noted that some individuals showed large spikes while others showed virtually none under identical conditions.
Stress is rarely the only driver of high cholesterol, and someone with an otherwise healthy diet, regular exercise habits and no family history of cardiovascular disease is less likely to see their cholesterol pushed into dangerous territory by a bad week at work.
What the research does suggest is that managing chronic stress is not just good for your mental health, it may be doing something useful for your lipid panel at the same time.
Whether your stress outlet of choice is exercise, sleep, a walk, or complaining about the traffic on the Federal Highway, the evidence suggests it is worth taking seriously.
