Task forces set up to probe Johor container blast, Butterworth train derailment

KUALA LUMPUR: The Transport Ministry has set up two special task forces to investigate the container explosion incident at the Port of Tanjung Pelepas (PTP), Johor, and the runaway KTM Komuter train incident at Butterworth, Penang.

Transport Minister Anthony Loke said that the establishment of these task forces was decided in the ministry's post-Cabinet meeting on Wednesday (Nov 19) to ensure investigations are conducted transparently, independently, and without influence from any parties.

He added that the meeting also reviewed the recent rainwater overflow incident at Terminal 1 of KL International Airport (KLIA).

"The container explosion on a ship during unloading operations on Nov 7 resulted in three fatalities and three injuries. The task force will examine all aspects of operations, safety compliance, and dangerous cargo handling mechanisms.

"Meanwhile, on Nov 8, an empty KTM Komuter train overshot the end of the track at Platform 4, Butterworth Station. The task force will assess the cause of the incident and the need for improvements to prevent recurrence," he said in a statement.

He added that the Land Public Transport Agency (APAD) has been instructed to open an investigation file on the train driver and take follow-up action once the investigation is complete.

Regarding the rainwater overflow incident at KLIA Terminal 1, Loke explained that it occurred during waterproofing works by contractor Teras Budi Resources Sdn Bhd under a RM5.27mil contract from April 15, 2025, to April 14, 2026.

Following the incident, Malaysia Airports Holdings Bhd (MAHB) issued a stop-work order to allow for internal investigations and immediate corrective action.

"I have also instructed MAHB to take firm action against the contractor for negligence causing the incident.

"The ministry emphasises that user safety and operational integrity are of the utmost priority. Operators in the national transport sector are reminded to fully comply with safety procedures and carry out continuous maintenance and monitoring.

"The ministry will provide updates on the progress of both task force investigations as they become available," he said. – Bernama
