Findings on gas pipeline fire raise critical questions

WHILE the diligent efforts by the various authorities to carry out the comprehensive three-month investigation on the April 1 Putra Heights gas pipeline fire (pic) are appreciated, the findings released on Monday have raised several critical questions that demand further public discussion.

For true transparency and to uphold public confidence in our vital infrastructure, the Alliance for a Safe Community urges the authorities to do the following.

1. Release the full investigation report promptly. This is fundamental for accountability, fostering trust, and ensuring that lessons are learned from this incident, especially given the pipeline network stretching over 2,600km across Peninsular Malaysia.

2. The report attributes the rupture to cyclic loading compounded by weakened soil due to underground water accumulation, implying a natural phenomenon affecting monsoon drainage and culverts. It would be important to understand the definitive process or proof used to establish that no human activities near the pipeline, such as inadequate drainage from adjacent developments or improper land use, compounded this water accumulation or directly impacted soil stability. This clarity is vital for developing targeted and effective prevention measures moving forward.

Heavy machinery was reportedly present at the site. What definitive proof can be provided to assure the public that this machinery was conclusively not a contributing factor to soil movement or pipeline stress in the area?

What types of surveillance programmes are currently in place to detect ground movement, soil instability or unusual water accumulation around pipelines? Why did they fail to detect the developing issue in this specific case? Were the personnel involved competent and adequately trained for this critical task?

3. Reinforce encroachment control and permitting: This is crucial because any unauthorised building, digging or heavy vehicle movement near a buried pipeline can disturb the soil around it. Clarification on activity control measures and enforcement around critical pipelines is essential to prevent future unauthorised or unsafe developments.

4. Issues like “cyclic loading” and “tensile overload” were mentioned in the investigation, and there were hints of a welding fault. Were any defects in the welding joints properly checked and analysed during the investigation? This is about making sure the pipeline was perfectly built and inspected right from the start.

The devastating fire underscored the critical importance of a rapid and effective response. The full report must clearly address:

> Fail-safe design and valve functionality: This means if there’s a sudden drop in pressure (the pipe operates at 50 bar), would it automatically trigger alarms and shut off the gas immediately?

> Response timeliness: Was the emergency response carried out exactly as trained and designed, or were there any deviations or areas for improvement in the immediate response?

> Command centre performance: How quickly and effectively did the emergency command centre activate and coordinate the response? Did it match what’s expected from drills and professional standards, or were there any shortcomings?

> Simulation testing: Were computer simulations (like transient flow analysis or surge analysis) conducted? These simulations help test what would happen if critical valves were shut off instantly when low pressure is detected.

> Maintenance and inspection frequency: Given that cyclic fatigue (where materials get weaker from repeated stress) is a gradual process, is a 10-year inspection interval (using intelligent pigging) sufficient for such a critical pipeline, especially with external factors at play? Most importantly, do the intelligent pigging tools used have the latest technology to detect subtle signs of cracks and fatigue that develop over time?

While we commend the thoroughness of the forensic analysis in determining the cause of the explosion, a truly holistic understanding necessitates a deep examination of all contributing factors, the efficacy of the emergency response and the robustness of ongoing integrity management.
