KUALA LUMPUR: An error in the procedure for lifting steel beams by the contractor caused the beams to fall and crush a vehicle at the LRT construction site in Subang last Thursday.
Deputy director-general (Operations) of the Occupational Safety and Health Department, Mohtar Musri said following findings of initial investigation, his department issued a stop work order on the contractor concerned.
