Break the silence over organ donation

ASK almost any Malaysian if they would save a life if given the chance, and the answer is an easy, unanimous yes.

Yet right now, more than 10,000 Malaysians – parents, youth, children – are sitting on organ transplant waiting lists, watching time run out as death nears.

It is a heartbreaking paradox. We boast world-class hospitals and remarkably skilled surgeons, yet our national organ donation rate remains low, hovering at around just one donor per million people.

We are facing a severe transplant shortage, but this is not a failure of our healthcare system.

It is a failure of our silence.

The disconnect isn’t that Malaysians lack heart; it’s that we refuse to talk about death.

Over 400,000 Malaysians have registered as donors on MySejahtera, which is a great step forward.

However, clicking a button on an app is only half the battle.

In Malaysia, the final consent always rests with the family.

When sudden tragedy strikes and doctors approach a grieving family in the intensive care unit, panic and uncertainty take over.

Without a prior, explicit conversation, most families default to a safe “no” – often driven by lingering, unfounded doubts about religious rules or medical procedures.

This is where the community must step in. Government campaigns alone will never fix this; the real work happens at our own dining tables, in local community centres, and across neighbourhood chat groups.

Major religious authorities, including the National Fatwa Committee, have made it crystal clear for decades that organ donation is a noble act of charity.

Faith is not the barrier. Silence is.

Signing up on your phone takes two minutes.

But spending two minutes telling your spouse, parents, or adult children why you pledged is what actually saves a life – or lives, because a single donor can rescue or transform up to eight lives.

We cannot wait for a national crisis or a hospital emergency to make us care.
