Vision for training specialists

THE Malaysian Public University Medical Deans Council is concerned and wishes to address a matter of utmost importance in our nation’s healthcare – the training of medical specialists in Malaysia via the master programme and “parallel pathway”.

Our vision is for Malaysia to be self-sufficient in training the specialists needed to meet the demands of our healthcare system not just in terms of numbers but also in the quality of medical expertise. Achieving this goal requires a concerted and collaborative effort to enhance the capabilities of local training providers.

While the 2023 Health White Paper focuses on the nation’s health as a whole, it underscores the critical need for a sufficient number of competent medical specialists to ensure the well-being of our population. This highlights the importance of addressing specialist training as a fundamental component of our healthcare strategy. The paper also encourages public-private initiatives (PPI), emphasising that all parties must collaborate.

The Medical Deans Council has embraced this call for collaboration and conducted three consecutive workshops among relevant stakeholders since last year. The following were among the outcomes:

1. Enhancing the curriculum: We are dedicated to improving the curriculum, particularly workplace-based assessments, by utilising our National Postgraduate Medical Curriculum to meet the Malaysian Medical Council (MMC) standards.

2. Introducing flexible training programmes: We are developing flexible training programmes that allow for varied placements of trainees and ensure that they receive diverse and comprehensive clinical exposure.

3. Revising entry requirements: Given the current limitations on training spots, we are revising the entry requirements to make them more equitable and reflect our capacity to train specialists effectively.

4. Enhancing collaboration with trainers: Strengthening collaboration with trainers in the Health Ministry and private institutions is a priority to ensure a unified, high-standard training approach.

5. Expanding partnerships for training centres: We are improving and expanding partnerships with the Health Ministry and private hospitals to increase the number of training centres available for specialist training.

6. Proposing joint committees: We suggest establishing joint higher and operational-level committees between the Higher Education Ministry and the Health Ministry to oversee the policy, quality, and implementation of all specialist training programmes in Malaysia, ensuring cohesive and strategic governance.

7. Improving training capabilities: Efforts are under way to enhance training capabilities within universities and teaching hospitals, ensuring they are well equipped to provide top-tier specialist education.

We must focus on the holistic development of our specialist training programmes. While discussions around the Medical Act are necessary, we must maintain sight of the broader issues. The Medical Deans Council is concerned that focusing on legislative changes may divert attention from improving training standards and infrastructure. We look forward to staying updated on the specific upcoming changes and appreciate that our input is being considered.

We call upon all relevant parties to come together and contribute to a comprehensive solution. This includes enhancing the curriculum, investing in state-of-the-art training facilities, and fostering an environment that encourages continuous professional development. By doing so, we can ensure that our medical specialists have the knowledge, skills, and experience to provide exemplary healthcare services to the Malaysian population.

This is a critical moment for us to act decisively and collaboratively. Let us work together to build a healthcare system that not only meets but also exceeds the expectations of our people. The future of Malaysia’s healthcare depends on our collective commitment to this cause.
