FRU truck crash: Lorry overloaded by 70% the cause of accident, task force finds


KUALA LUMPUR: The lorry that crashed into a police truck and killed nine Federal Reserve Unit (FRU) personnel in Teluk Intan, Perak was overloaded and the main cause of the accident, says the Transport Ministry.

The ministry said in a statement on Friday (Oct 31) that these were the findings from the Road Transport Safety Investigation Special Task Force established to investigate the cause of the accident.

In the incident, nine FRU personnel were killed, two seriously injured and seven others sustained minor injuries during the accident between the police truck and a tipper lorry on Jalan Chikus-Sungai Lampam, Teluk Intan at 8.50am on May 13, 2025.

The police truck was carrying 15 FRU personnel from Unit 5 Ipoh.

"The task force found that the tipper lorry carrying gravel exceeded its permitted gross vehicle weight load by 70%.

"This situation caused the vehicle to go out of control, resulting in the collision with the FRU truck which was in a convoy returning to Ipoh, Perak.

"The report explained that no mechanical failure was detected in the braking or steering systems.

"The accident was caused by operational and compliance factors, including weaknesses in the lorry operating company's internal controls and the absence of a functioning global positioning system (GPS) monitoring system," it said.

The ministry also said a total of 16 safety recommendations were submitted, covering six main areas.

"This includes improving Occupational Safety and Health Industry Code of Practice (ICOP) safety compliance, the Land Public Transport Act 2010 as well as the safety features of government vehicles such as installing seat belts for all seats.

The ministry said the investigation is not intended to assign blame or determine liability for any party, but rather to strengthen road transport safety policies so such tragedies do not recur.
