PETALING JAYA: Incorrect medication and labelling, and even omission of medicines, are among the common mistakes that the Health Ministry has detected in the treatment of patients nationwide.
Its pharmaceutical services division's Medi-cation Error Reporting System showed that most were near misses errors made but were rectified before impacting the patients due to safety measures implemented.
